Packing Lists
Ready to start gathering your supplies? Here are the 2021 trek packing lists. More information was provided at the trek workshop on April 10. Follow the link or go to events to review that information.
A note about electronics: Youth participating in trek should not bring electronics with them, including phones, electronic games, and cameras. The ma's and pa's may bring a cell phone with the expectation that the phone is used to enhance the experience for the youth (by using it to take pictures of family moments, playing accompaniment to hymns, or using any available cell service in the case of an emergency.)
Permission and Medical Form
Everybody (youth and adult) participating in trek must fill out and return a permission and medical release form. This can be done completely digitally. The form has digital signature capability for the participant (and parent, as required for youth under 18 years old.)
Please fill out and return this form no later than Sunday, March 28, 2021. The form may be returned by e-mail to tss.trek@gmail.com. All youth must bring their own tent or cot to sleep on. When e-mailing your release form, please indicate if you will be bringing a cot or a 1-2 man tent. If you have neither and need help finding a tent to bring, please indicate. Also indicate shirt size of the participant (Adult S, M, L, XL, etc.)
Instructions to sign digitally: Right click on the download button and choose "save link as..." from the drop-down menu. From the 'save as' window, navigate to the desired location you would like to save the form and save. Open the document with Adobe Acrobat. If you open from a web browser, the signature fields won't be available. Once open, fill out the form completely (including the 'ward' field under the Event Details section.) To sign, click on the signature block (participant and parent, as required.) Choose or configure a digital ID (saving as a file works fine), input your ID PIN, and then save the signed document. Once your form is filled out and signed, please rename as follows if possible: Lastname,Firstname_permission.pdf and send to the email address listed above.
There are a couple of small fields that may not have sufficient space to hold all of your necessary information. When e-mailing the form, you may add the remainder of your notes to the e-mail text or send an additional document (pdf or word documents are fine).
You can sign with the free Adobe reader. If you do not have the software, you can download for free at https://get.adobe.com/reader/ Watch out for all the free offers on the page (you will probably want to uncheck boxes) and click the desired download box (you just need the free reader, not the pro edition.
If you are unable to send your form digitally, please turn in to your ward YW presidency or Bishopric. They will need to either scan it and send to the above address, or physically deliver it to Nathan Powell (trek secretary) in the Mt. Fagan Ward.
